Application

Admissions Process:

We encourage parents/guardians to schedule a tour by contacting our school office, PJASecretary@gmail.com or 315-468-0117.

After prayerful consideration, please submit an application and other required entrance materials--copy of birth certificate, recent immunization records, and recommendation for admission (K-8th grade only).

The Admissions Committee will review your application and make a recommendation to the school board. The school will then notify the parents/guardians in writing with a decision regarding acceptance.

Once a student is accepted, complete the following applicable forms linked below:

Print and complete ONE form PER FAMILY:

  1. 48 Hour Pesticide Notification Letter (required by NY State) 

  2. Emergency Contact Card (this will be provided to you during registration)

  3. Nutrition Guidelines

  4. Field Trip Permission Form


Print and complete ONE form PER STUDENT:

Print and complete ONE form for each student in Grades 2, 4, and 7:

In December of each year, if the student plans to return the following school year, print and complete this form.
